docs(provenance): 补充上游派生技能章节与处置记录

- 新增 §3.1 上游派生技能:逐包列出 5 组派生关系
  (speckit-skills / python-skills / devops-skills / stitch-skills / agent-skills)
  及各自上游来源、许可与版权方
- 记录 speckit-skills 的 AGPL-3.0 链条处置:改为从 MIT 上游 github/spec-kit
  重新生成,并说明 speckit-baseline 的原创改写
- §4 包清单更新派生计数与章节引用
- §7 已知缺口记录本次全部修复项(17 个 MIT 归属、2 个 Apache 归属、
  10 个 speckit 重生成)
