refactor: sync consumer prompts with new mainline

- SKILL.md Step 5.4: replace vague 向量索引失败 with projection_status.vector=failed
- test_prompt_integrity.py: add snapshot_manager.py to KNOWN_DELETED_FILES
- Verified: context-agent.md has no snapshot refs, data-agent.md correct
