Merge remote-tracking branch 'origin/main' into garrytan/design

Resolves conflicts in 7 files: CHANGELOG, README, TODOS, VERSION,
gen-skill-docs.ts, skill-check.ts, skill-validation.test.ts.
Regenerated all SKILL.md files after merge.
